2nd Level ( Advanced USG ) Ultrasound


Ultrasound is one of the most important tests that has enabled the early diagnosis of many problems in the follow-up of both normal pregnancies and problematic pregnancies during pregnancy. Ultrasound in pregnant women can be considered as an examination of the baby in the womb. In this respect, it is recommended to perform ultrasound evaluation at least once every 3 months. The first ultrasound performed during early pregnancy is particularly important, because in this way, the gestational week of the baby is determined exactly and subsequent follow-ups are evaluated accordingly, and it can be easily understood by ultrasound whether the early pregnancy is a normally located pregnancy or an ectopic pregnancy. During this examination, various body measurements of the baby are made, the estimated weight is calculated, the amount of fluid is measured, and it is determined whether the findings are compatible with the gestational week. When necessary, detailed examination and follow-up with advanced ultrasound are performed by experienced physicians.

Detailed ultrasound or detailed ultrasound or advanced (second level) ultrasound are synonymous. 3D ultrasound or 4D ultrasound do not mean the same thing, but they are often mistaken for the same. During the detailed ultrasound, the baby’s face, hands and feet can be shown in 4 dimensions, but the detailed ultrasound is mainly done in 2 dimensions. The important thing in detailed ultrasound is not whether the ultrasound device is 3 or 4 dimensional, even if it is not 3-4 dimensional. What is important in detailed ultrasound is the evaluation of an experienced specialist (usually a perinatologist, obstetrician or radiologist).

In normal pregnancy follow-up, fetal anomaly screening is performed between 17-22 weeks by obstetricians. Fetal anomaly screening (first-level ultrasound) is performed in every normal, risk-free pregnancy to investigate the presence of a birth defect (anomaly) in the baby. When a risk or suspicion is detected in fetal anomaly screening, the patient is referred for detailed (second level) ultrasound examination for further examination. Here, the perinatologist examines again and gives the family detailed information about the baby’s condition.

Detailed ultrasound can be performed between 19-24 weeks, but often 17-19. applied in weeks. The review takes about 20-30 minutes. Sometimes, if there is a suspicion, ultrasound is performed again after 3-4 weeks, but if there is no suspicion or risk, it is performed once in a normal pregnancy and is not done again.

In detailed ultrasound, the baby’s head, brain, spine (waist opening), chest, heart, abdominal organs, stomach, intestines, kidneys, arms-legs (introversion in hands and feet), fingers (missing-excessive), face, (cleft palate-lip) etc. every organ is examined in detail. In addition, the baby’s water, placenta (baby’s partner) is evaluated.

Detailed second level ultrasound is often done at 17-18 weeks, these weeks are early to evaluate the heart, it cannot be evaluated very well. If necessary, the heart is most clearly evaluated by fetal echocardiography at 21-22 weeks later.

Is every anomaly determined with detailed ultrasound?

Even if detailed ultrasound is performed by the most experienced specialists in the best centers, it can never detect 100% of the anomalies in the baby. Approximately 70% of anomalies can be detected. Because some anomalies do not show signs to be detected on ultrasound, some anomalies develop in later months after ultrasound is done.

Normally, approximately 2-3% of all pregnancies have anomaly (congenital defect). In identical twins and consanguineous marriages, this rate increases to 4-5%.

What pregnant women need to know when coming to ultrasound
It is not necessary to come hungry for ultrasound in pregnant women. You should have the results of the previous ultrasound and examination, the pregnancy follow-up form and your file with you.
